Project Introduction
Forum hosts a range of projects in Hull to support individuals, with teams based in the Hull office, GP practices and community settings. We also facilitate the Hull Social Prescribing Network for any staff or volunteers working in this area
Waiting Well Hull supports people who are on waiting lists for non-urgent care to improve their health and wellbeing by getting active. The Primary Care Link Workers help patients at the Acclaim and Haxby GP practices in Hull to find activities in their communities.
What we do
Social prescribing teams have helped people to:
- Find outdoor activities that suit their needs and mobility
- Manage symptoms and overall health while waiting for procedures
- Maintain independence
- Find local groups that improve their social networks and find new friends
- Gain the confidence needed to get out and about more and improve health
- Reduce stress and improve emotional wellbeing
- Visit the doctor fewer times by getting help to visit other places of support
