Project Introduction
Forum hosts a range of projects in Hull to support individuals, with teams based in the Hull office, GP practices and community settings. We also facilitate the Hull Social Prescribing Network for any staff or volunteers working in this area
The Primary Care Link Workers help patients at the Acclaim and Haxby GP practices in Hull to find activities in their communities.

I wish to praise the Social Practitioner for how she has helped me and still is. They puts me at ease and so helpful with my depression I only now suffer at times and listens then helps but has never rushed or pushed me into things that take me time to do. They were kind and very helpful and at the end of a phone if i'm in need.

What we do
Social prescribing teams have helped people to:
  • Find outdoor activities that suit their needs and mobility
  • Manage symptoms and overall health while waiting for procedures
  • Maintain independence
  • Find local groups that improve their social networks and find new friends
  • Gain the confidence needed to get out and about more and improve health
  • Reduce stress and improve emotional wellbeing
  • Visit the doctor fewer times by getting help to visit other places of support
