Project Introduction
Forum's Community Health Connectors work with communities across Hull to ensure that public health messages are developed in a way that makes them accessible.
Based in community settings across Hull, the team provide one-to-one, face-to-face support to help people find activities in their area. They can link people with advice, mental health services, healthy lifestyle activities, volunteer opportunities and more. They work in a holistic way that takes into account individuals' needs, goals and existing networks.
What we do
The Community Health Connector team work with communities and public health teams across Hull to:
- Develop public health messages that are based on the needs of different communities
- Ensure communities can have an active voice in service design
- Help people to get more involved in the things that matter to them
- Undertake specific engagement when required
