Project Introduction
Forum's Community Navigators work with individuals to improve their health, wellbeing and social networks
Based in community settings across Hull, the team provide one-to-one, face-to-face support to help people find activities in their area. They can link people with advice, mental health services, healthy lifestyle activities, volunteer opportunities and more. They work in a holistic way that takes into account individuals' needs, goals and existing networks.

What we do
The team have supported thousands of people in Hull with the following:
- Finding social activities like lunch clubs and walking groups
- Achieving personal health-related or other goals like getting active or getting online
- Links to volunteering opportunities
- Giving time to talk through challenges and needs
- Support to navigate things like housing, employment and health services
- Help to find courses and activities to develop new skills
