The Liaison and Diversion (L&D) service is a partnership between Leeds Community Healthcare NHS Trust (LCH) and Community Links, funded by NHS England to deliver L&D Services in Hull and East Riding, Grimsby, North and North East Lincolnshire.

Rooted in Hull aims to promote and educate local people about food: how to grow veg, nutritional content, health diet choices, as well as basic construction, gardening and other agricultural volunteering tasks.

The service is for people who have had a stroke or transient ischaemic attack (TIA) and people at high risk of a stroke within the Hull area.
