City Health Care Partnership (CHCP) Community Service Information

City Health Care Partnership (CHCP) have recently been commissioned to deliver community services across both Hull and the East Riding.

Community Services cover a wide range of health and social care activity including:

• Community nursing
• Community therapies
• Planned care
• Anticipatory care / frailty
• Urgent / unplanned care
• Sexual health
• Children and Young Peoples Services

CHCP are currently in the process of reviewing these services and are keen to obtain stakeholder feedback to inform the process. This can be done via the following on-line form:

The aim of the recommissioning is to move towards more equitable community services, based on local need. The overall aim is to develop more streamlined and effective pathways of care and a more integrated approach to service delivery.

This initial feedback will be followed by a stakeholder event, which is being planned for November 2023.
