Healthier Together Survey

To help you when the children you care for are ill, or have an accident, Humber and North Yorkshire ICB are developing an information website. The website will have information about what to do when a child becomes ill or has an accident, how you could look after them at home, when to get help from health services, and who to contact. The website we are developing for our area has been successful in other areas, and is at an early stage of development in our area.

We would like your views on the early version of the website to make sure the content and function of the site will be useful to you when you need it most. We will be using the views of parents and carers as the website develops, and after it is live to make sure it meets your needs.

The following questions are about specific elements of the site. There will also be questions about you and your family to make sure the site meets the needs of as many different groups of people as
possible. You will also be asked if you would like to be involved in the development and improvement of the site in the future.

The questionnaire may take about 20 minutes to complete, you don’t have to answer all the questions at once, return and change anything until you press submit.
