Community Services Information Event
As you may be aware City Health Care Partnership (CHCP) have recently been commissioned to deliver community services across both Hull and the East Riding.
Community Services cover a wide range of health and social care activity including:
- Community nursing
- Community therapies
- Planned care
- Anticipatory care / frailty
- Urgent / unplanned care
- Sexual health
- Children and Young Peoples Services
The aims of the recommissioning is to move towards more equitable community services, based on local need, to develop more streamlined and effective pathways of care and a more integrated approach to service delivery.
As part of the commissioning process CHCP are reviewing these services and have been working with stakeholders to inform the process.
As part of this process, we would like to invite you to a stakeholder event at The Hull History Centre on Wednesday 6th December, 9:30am – 12:00pm.
